To improve the quality of life for people at risk of losing their independence.

LifeStream Services is the Aging and Disability Resource Center for East Central Indiana. We provide services and programs to help seniors and people with disabilities remain independent. We also support caregivers who need help maintaining the health, dignity and independence of their loved ones.
Program Description

Attendant Care: This service provides a trained, caring attendant to assist a disabled or disoriented client.

Caregiver Support: Our CareGiver program helps those who are taking care of family members as well as providing support for grandparents who are raising their grandchildren. We offer respite care, support groups, resources and real help for caregivers.

Guardianship: We have several years' experience in helping those who can no longer take care of their private affairs. Our guardianship and asset management services help people who don't have family or friends to help take care of their well-being.

In-Home Care and Options services: Our care managers help clients connect with the care they need and deserve. Our In-Home Care Managers and Options Counselors assess clients' needs, identify funding sources for services and provide our clients' choices of in-home service providers in their area, authorize delivery of service and follow up with clients to make sure they are satisfied and getting the level of care they need.

Information and Referral: We are the regional ADRC for East Central Indiana, and callers can call for information and referral for a wide variety of health and human service groups and agencies. Our trained counselors can assist in helping to find the right information to fit your needs. Our Information and Referral service is also the entry point to services.

Making Your Voice Heard (Advocacy): We take a leadership role in advocating on behalf of seniors and people with disabilities. You can help, too, through by calling or writing your government officials or by letting us know what concerns you have.

Respite Care: Arrangements can be made to give caregivers some time away so they can take care of their own needs or take care of errands for the client. Respite care is an important part of the overall care plan.

Transportation: The New InterUrban is a rural transportation service open to people of all ages in Delaware, Blackford, Grant, Randolph, Jay, Madison and Henry counties. Call us (888) 589-1121 to schedule a ride or receive more information.

Wellness Activities: LifeStream sponsors programs such as the annual Aging Well conference, Senior Games, LifeTrail exercise stations and other activities to encourage wellness for older adults and people with disabilities. Medicaid Waivers: Medical waivers (TBI and A&D)
