Program Description
Online Training Modules - The Transition Coalition provides research-based online training modules for professionals and others involved in transition planning. These modules are developed using current research in transition and professional development, and they are used by practitioners and university faculty across the country.

Publications - We offer a variety of free publications for download. All materials are current and easy to read. Some items available include:

A how-to manual for creating a community-based 18-21 program that was developed by teachers
A customizable brochure about transition
A customizable brochure about age of majority
A booklet about different people involved in transition and the important roles they play

Resources - The Transition Coalition website is chock-full of resources and materials that focus on transition-related topics. These include:

Online mini-modules that focus on topics such as person-centered planning and individualized funding
A searchable database of more than 100 community-based 18-21 programs across the country
Quality Indicators of Exemplary Transition Programs Needs Assessment—a self-assessment to determine and help you prioritize the most critical transition program needs

TC Self-Study - This is a team-focused 12-week professional development process with materials and online support. The Transition Coalition Self-Study can improve special education compliance and enhance the abilities of secondary special education staff to improve outcomes for students with disabilities.

Professional Development Implementation - The Transition Coalition works with LEAs and SEAs to help them identify professional development needs and develop and implement strategic professional development initiatives focusing on secondary transition.
