Mission
The mission of Area IV Agency on Aging and Community Action Programs is to foster a better quality of life for individuals, families and communities. The agency aims to maximize and make the best use of all financial and human resources, with compassion and respect for the dignity of each individual served and with stewardship that reflects commitment to accountability, integrity, and quality services.
Program Description
We are the Aging and Disability Resource Center (ADRC) for our area.

Area Agency on Aging is the required point of entry for Indiana's CHOICE program (Community and Home Options to Institutional Care for the Elderly and Disabled) , in addition to Medicaid waivers, Social Services Block Grants and Title III Services. In addition, since the Agency is a Community Action Program, a variety of services are offered that may be of interest to families with special kids' needs such as energy assistance and home weatherization.

The case management services include:
  • client initial screening and intake;
  • assessment of client needs;
  • determination of service and/or care needs;
  • determination of vendor or service provider;
  • service arrangement or linkage with resources;
  • on-going monitoring, review, coordination, and care plan modification client discharge/program termination.
Case managers make home visits to determine personal care and other needs, develop care plans, and coordinate a variety of in-home and other needed services. Supportive services may include:
  • home-delivered meals;
  • home health aides;
  • homemaker services;
  • adult day care;
  • transportation;
  • respite care;
  • attendant care;
  • home modifications;
  • adaptive aids and devices.
Case managers also perform financial assessments to make sure that individuals have access to any subsidized service or benefit for which they might qualify. Information gathered during a financial and/or needs assessment is confidential.
