Mission
Steering youth with special health care needs toward successful adulthood.
Program Description
The transdisciplinary team includes physicians, nurses, social workers, parent liaisons, a program manager, and administrative support. Services include:
  • Evaluation and development of individual health plan which includes portable medical summary to be shared with other providers and transition plan.
  • Consultative services for medical, rehabilitative, genetic, vocational, social, sexual, self-management, independent living, health care financing, long term planning, parenting, and community resources.
  • Care coordination that provides advocacy, education, implementation and evaluation of service arrangements.

The Transition Support Clinic is funded through the Indiana State Department of Health, Maternal and Children's Special Health Care Services.
