Program Description
We are the Aging and Disability Resource Center (ADRC) for our area.

Description: Area 11 Agency on Aging offers case management services to people in their area (Bartholomew, Brown, Decatur, Jackson and Jennings Counties).

Area Agency on Aging is the required point of entry for Indiana's CHOICE program (Community and Home Options to Institutional Care for the Elderly and Disabled) and a variety of Medicaid waivers.

Case managers make home visits to determine personal care and other needs, develop care plans, and coordinate a variety of in-home and other needed services. Supportive services may include:
  • home-delivered meals;
  • home health aides;
  • homemaker services;
  • transportation;
  • respite care;
  • attendant care;
  • home modifications;
  • adaptive aids and devices.
Case managers also perform financial assessments to make sure that individuals have access to any subsidized service or benefit for which they might qualify. Information gathered during a financial and/or needs assessment is confidential.

Ramps to Freedom through Thrive Alliance and Servants at Work are working together to construct and install high-quality wooden ramps for individuals in need of safe access into and out of their homes. Applications for Ramps are now being accepted.

First Steps Southeast is a direct service of Agency covering 25 counties. Providing access to services for children birth to three years of age who are at risk. The case management services include:
  • client initial screening and intake;
  • assessment of client needs;
  • determination of service and/or care needs;
  • determination of vendor or service provider;
  • service arrangement or linkage with resources;
  • on-going monitoring, review, coordination, and care plan modification;
  • client discharge/program termination.
