Program Description
Aging and Disabled Resource Center (ADRC) - This is the entry point for people seeking help. Trained specialists provide information and options to questions about senior issues such as Medicare, housing, transportation, equipment, financial assistance and home health care. Have questions? Senior, Disabled, Rural Transportation - (812) 232-2675 Transportation for medical and nutrition needs is available to persons 60 years and older on a donation basis in Parke, Vermillion and Vigo Counties from 8:00 am to 4:00 pm Monday through Friday. Disabled persons under age 60 may qualify for programs that assist with transportation in those counties. Rural public transportation is available in Vigo Counties on a fee basis. Nutrition Program - The Nutrition Program provides hot noon meals at community dining sites to persons age 60 and over. Each site offers fellowship and educational programs. Some sites also offer crafts, games and other social activities. Contact us for a site location and the hours of operation. In addition, home delivered meals are available to homebound persons age 60 and older or disabled (applicable to funding guidelines). McMillan Adult Day Care Center - McMillan Adult Day Services, 489 2nd Ave., Terre Haute, IN 47807, offers a unique and affordable alternative to long-term care for families of adults who cannot be left alone during the day, but do not require 24 hour nursing care. Trained activities specialists offer a full activities program, designed to meet the needs of older adults and to maintain their or improve their level of functioning. A nurse on staff monitors clients and provides healthcare as needed. Individualized care planning insures personalized services for each client. Center hours are 7:00 am to 5:30 pm., Monday through Friday. Case Management - Trained personnel visit the homes of their clients to discuss and monitor the needs of the individual. Some funding requires quarterly visits to monitor needs and level of care for the in-home service. Case Managers follow-up with clients ensuring that needs have been met and have not changed.

Pre-Admissions Screening - Personal screening of individuals who are at home and are needing nursing home placement. Call the ADRC 812-917-3145/800-489-1561

Preventive Health - Various programs on wellness, promoting exercise, fitness, proper nutrition, vision care, home safety, mental health, smoking cessation and the safe use of medications.

Ombudsman - An advocacy program which assists with resolving problems between nursing home residents, administration, staff members and families. Contact: John Turner, Ombudsman, 812-238-9455 ext.#201

Covering Kids and Families of West Central Indiana - Indiana Navigator assists with Marketplace, Hip, Medicaid enrollment.
