CICOA Aging & In-Home Solutions empowers older adults, those of any age with a disability and family caregivers to achieve the greatest possible independence, dignity and quality of life.
Program Description
The Aging and Disability Resource Center (ADRC) offers information and resource referral to individuals. Phone Options Counselors provide callers with information regarding long-term support services, options counseling and community resource referrals. CICOA also offers care management services to people in Marion and the 7 surrounding counties. Area Agency on Aging is the required point of entry for Indiana's Medicaid Waivers; Aged and Disabled Waiver and TBI waiver as well as CHOICE program Services. The care management services include:

a) client initial screening and intake;
b) assessment of client needs;
c) determination of service and/or care needs;
d) determination of vendor or service provider;
e) service arrangement or linkage with resources;
f) on-going monitoring, review, coordination, and care plan modification
g) client discharge/program termination.

Care managers make home visits to determine personal care and other needs, develop care plans, and coordinate a variety of in-home and other needed services. Supportive services may include:

1) home-delivered meals;
2) attendant care;
3) homemaker services;
4) adult day services;
5) transportation;
6) respite care;
7) Adult Family Care;
8) home modifications;
9) adaptive aids and devices.

Care managers also perform financial assessments to make sure that individuals have access to any subsidized service or benefit for which they might qualify. Information gathered during a financial and/or needs assessment is confidential.

CICOA offers the CareAware program which provides support to caregivers through one on one support, education/awareness activities, presentations/workshops and linkage to community resources.  www.careawarejourney.org

Freedom Transportation Vouchers - Qualified individuals may purchase up to 10 vouchers per month from CICOA at $6 per voucher. Each voucher is good toward a one-way trip, for any purpose, from an approved list of transportation providers. Participants arrange transportation directly with the transit providers. Depending on the distance involved, providers may require two vouchers per one-way trip. Most providers are willing to transport participants across county lines. One attendant may accompany a participant at no additional charge. There is a limit on the number of vouchers CICOA can issue per month. When this limit is reached, clients will be asked to call back the next month. Active and waitlisted clients will have the option to purchase full-price vouchers at $20 per voucher.   For more information, visit http://cicoa.org/services/way2go/new-freedom.html
